If you are about to participate in a Challenge Applications program, please read the Program Disclosures and Information document below to learn about our philosophy, how we offer flexibility and how you can prepare.
Then, you must complete the Online Participant Release Form as it provides important medical information in the event of an emergency. Challenge Applications takes HIPPA very seriously and we use the form to facilitate timely and appropriate medical response in case of an emergency. This form uses an SSL certified connection.
